Deciding between the sling operation and the artificial sphyncter
I am a 64 year old male and very active. After a prostatectomy for cancer 3 years ago and a course of radiotherapy - I am now inconveniently incontinent - 2 pads perday - I leak when I am active - walking, gardening, lifting - but can hold a full bladder. I have consulted two very good surgeons- one advises me to have the Artificial sphyncter (and dislikes the sling operation) the other thinks I the sling is the answer should have the sling. Has anyone had any longer term experience with the sling to help me make this decision.
Richard
Hello Richard,
You ask good questions. Please see this article by Dr. Sobol:
A New Treatment for Male Stress Incontinence
http://www.healthcentral.com/incontinence/c/52/12893/male-stress
In particular, note where she writes:
"Following suit with the minimally invasive type of slings that women have been able to enjoy recently, there is now the trans-obturator sling for men. The incision is similar to that for the older sling but the procedure involves only placing a mesh-like material against the urethra to increase resistance. The complication rate is pretty low, and the procedure itself is easier for the surgeon. The results are comparable or better to the older sling that used bone anchors."
I hope this is helpful to you.
Best,
Maria

I have about the same incontinence that you do and I have never considered surgery. I use pads sometimes but am now using the McGuire urinal most of the time. I also use external catheters with a leg bag sometimes (Medicare will cover those).
Surgery always has its downside and the benefits are not guaranteed.
