I'm afraid I can't think of any very helpful or specific advise, but you may want to talk to your doctor about the possibility of going to a pelvic-floor physical therapy specialist (your doctor will need to prescribe physical therapy and recommend someone who works with the pelvic floor) - this may help you gain some control over the pelvic floor muscles and the problems you mentioned in your second question.
