Wow Im sorry you had such a hard time with this, but congratulations on finally getting it. I had to get my Intermittent FMLA this past April and I was very lucky that my POE didnt give me too much of a hard time.

I put in a request but never got a response from HR. This was almost a year ago. To make matters worse, they expect you to re-apply for it every 3 or 6 months- I forgot exactly how long it is approved for (if approved). I find it creepy that the doctor's office charges a fee to fill out the form, why can't they submit the cost to insurance and charge a co-pay (office visit)? I am in the same predicament of overusing sick time and vacation while sick time (unplanned vacation time). What can be done besides hope that healthcare reform addresses some of these issues!

Thanks!  I am excited.  I only got it so they won't write me up for calling off.   They are good for doing that here.   So I wanted to be protected.  Also if I run out of sick time they can use my other days.
