To make matters worse, they expect you to re-apply for it every 3 or 6 months- I forgot exactly how long it is approved for (if approved). I find it creepy that the doctor's office charges a fee to fill out the form, why can't they submit the cost to insurance and charge a co-pay (office visit)? I am in the same predicament of overusing sick time and vacation while sick time (unplanned vacation time). What can be done besides hope that healthcare reform addresses some of these issues!

Wow Im sorry you had such a hard time with this, but congratulations on finally getting it. I had to get my Intermittent FMLA this past April and I was very lucky that my POE didnt give me too much of a hard time.
