I need a local Headache specialist/ Neurologist here in NC to work with my Headache Specialist- who's in Dallas, TX.

So, I'm going to see a new doctor on weds, and before I get there, I have 15 pages of information about myself to fill out. It's funny, as I'm raising the dose of my gabitril, and I have some side effects going on- nothing serious, but, I quote "Have you had any of these reactions in the past month."  Well. Yes. Sleepy, gaining weight from the steroid, fuzzy vision, abnormal sleeping, and some other fun things...

So, I'll fill out what I can, and explain why I used six colors of ink to explain what happened on the medication taken page. 

They keep asking me what I do for a living. How demoralizing to have to keep writing "layed off."
