Teri,
A suggestion on the Questions page, would be to have the 2nd side for Responses from Doctor, then maybe a 7th page for pharmacist & other health care professionals.

At one point, the entire last page was for questions for and answers from your doctor, and there was a separate page for pharmacists and others. People wanted fewer pages, so I condensed them as they are now.
Really, no matter how I set it up, some people will want it done differently because we all do things a bit differently. For now anyway, I don't plan any changes.
