Long-term consequences of frequent triptan use?

By Ask the Clinician, Health Guide Monday, August 16, 2010
Full Question: I've been dealing with migraines since I was a young adult; about 30 years.  A doctor prescribed Imitrex in 1998. It was like a miracle for me; especially after realizing how many days and events I'd previously missed or if present, could not fully enjoy due to migraine an...

I wish someone would do a study on the long-term daily use of triptansI! I am extremely sensitive to medications and started having migraines INfrequentqly when I was 38. Then they progressed to every month for 2 days before my period when I was in my early to mid forties. Then progressively hitting me back to back...four days at a time and more than once a month...AND at that time there were no triptans...it was hell! When Immitrex came out, it helped a lot at first but then I noticed it was not working as well...but I was not having migraines daily at that time. Around age 49, my migraines became daily and Maxalt or Zomig were the only triptans that worked for me. I have tried each class of preventatives, one for 2 years and I still had to take Maxalt every day because the preventative didn't help at all. The other preventatives I have tried had devastating side effects immediately, i.e. worsening migraines with one and suicidal feelings with nostop crying for no reason at all with another....and, I am an extremely happy person if it wasn't for these inherited, terrible migraines! So, basically, I have been on Maxalt or Zomig everyday since I was 49 (I'm 61 now) and sometimes take 2 to 3 per day. My liver enzymes are PERFECT, my blood pressure is pretty much 100/70 or less, no cholesterol problems, etc. I know I'm not having rebound headaches...I've proven it several times by: eating only meats and vegetables and within 11-14 days the migraines stop. Did this for 4 months one time (very hard to do...no carbs) and took no meds and had NO migraines or headaches of any kind even when I was stressed. Also I had a lower facelift 6 years ago after which I had no migraines and no meds for 3 months. Three years ago, I applied to Mayo Clinic for help and after submitting family history, MRIs, list of everything I've done, Mayo said they didn't think they could help me at the time. Then, a year ago, I had nerve decompression surgery specifically for migraines by the top surgeon in the country and it didn't work at all...and we paid out of pocket and traveled to do this. I have a really bad family history (on my Mom's side) of migraines with some relatives having them daily. Maxalt gives me a life. I would love to be in a study designed to see what the long term effects are since I'm probably the best candidate for the study having taken them for so long everyday. I look at the side effects profile of all preventatives and think....why would I want to put on weight (which I did with one preventative) if diabetes runs in my family AND my blood sugar level increased dramatically after being on that preventative for 2 years? Why would I want increased liver enzymes if mine are GREAT right now? And, you have to take such high doses of these preventatives! This just seems insane! Mayo and others have found (in 2010) that there is a genetic defect in families with migraine that causes glutamate to build up in the brain, this causing the end result of swelling of the blood vessels. What I really need is a glutamate blocker! Now THAT I would try because it would be specifically targeted for migraines.
