Q: 8 year old boy who seems to have NDPH, what can I do to help him

My son has been sick with a tension type headache for over 6 months now. 

It started one night when he complained of a headache and went to bed early, when we checked on him he had a fever which with childrens advil went away.  The headache never did and he has been in pain 24/7 ever since.  The pain is located at the crown of his head (where you would wear a beanie) and feels like pressure.  He is always very tired, and complains of mild nausea.  He is also pale and at points can be as white as a ghost. He has had a few migraine episodes where he has been in a lot of pain and threw up and couple of times has felt lightheaded with nausea. 

He also had a few nightmares (a first for him) in which he acted almost as if he was hallucinating a giant spider in his room since it took a long time for him to believe there was no spiders in his room.  He had this dream a few times and for a couple of weeks had to sleep in my husband and my room since he was to scared to be in his.  He still will not let us close his bedroom door at night and will not sleep near the wall even though these dreams happened 4 months ago.  Doctors feel that this was stress related due to the ct scan and mri and him not feeling well.

He has had a ct scan and mri (not sure if an mra was done at the same time) which found a achnoid cyst and damage from a silent stroke which doctors think happened between birth and 2.5 years of age. This stoke effected his right sides motor skills.  They do not feel feel that these findings are the cause behind his headache. 

He has tried the following drugs with Topiramate, Naproxen, Sandomigraine and 3 days of prednisone with him only experiencing side effects and no pain relief.

When I read about NDPH it sounded exactly like what my son has complained about...sudden onset without ever complaining about having headaches in the past and never ending tension type headache.  He also has hypermobility due to the stroke and a family history of migraine problems (maternal grandfather had sever migraines). 

I was wondering what test or treatments I can look into to help my son.  I have seen lots doctors with 3 being pediatric neurologists and they all had differents ideas the last being he was stressed and hinted that it was all in his head.  He is a really layed back child who does not let much bug him and is not one to complain about pain unless it really hurts. 

I am so frustrated which is why I have decided to ask other parents or patients for advice.

Hi Angie,

 

It sounds like it is time to get your son to a "true" MIgraine specialist, rather than neurologists who say they are headache specialists. Now don't get me wrong, neurologists are fine doctors, but are unable to be experts in one area because they treat so many different conditions like MS, stroke and epilpesy. Migraine specialists are just that, experts in one area - ours -Migraines and headache disorders. With your son suffering so much, it really would be a good thing to get him to the expert.
