Suffer from Migraine, crohn's&chest wall pain. Speci/Prim doc differ-take oxy4x's/just 4breakthru?

On 66% of days, the Fiorinal 3 works and keeps me function as a specialist who did early days research at Cleve Clinic on Crohn's states that is gud 2 take as the codeine helps the migraine, pain from crohn;s, and the  chest wall pain from my lung surg., barb slows digest tract, caffeine keeps me awake and able to lead functional life. Primary doc believes i shud take the oxycodone on reg basis so extreme pain won't occur... my f3 and oxy intake has decreased over the past 14 yrs, so oxy 4X's/day or only use for breakthru where i take basically same amount per month. this is only thing they differ on...

Yikes! This is one you're rally going to have to work out with your doctors. If we were talking about Migraine only, doctors would tell you that you shouldn't take Fiorinal 3 OR oxycodone more than two or three days a week to avoid medication overuse headache, aka rebound. You can read more about that in Medication Overuse Headache: When the Remedy Backfires.

 

There's also evidence that ANY use of opioids or barbiturates and frequent use of triptans and NSIADs are associated with increased risk of transformed Migraine. See Transformed Migraine - Risk Increased by Some Medications for more information on this.

 

However, you have other pain issues to deal with, so that changes things. Are you being treated at the Cleveland Clinic? If you are, I think one of the best things you could do is see one of the Migraine specialists there, AND have all of your doctors there sit down for a consultation about your case. This is best done with you present, either in person or by conference call.

 

In any case, your doctors should be talking with each other and with you to coordinate your care. That's part of their job. If they're telling you conflicting things, and leaving you in the middle, that's just not right.
