what is the number for Creatinine clearance;hypertension

Dear Doctor:

 

My doctor told me I have Chronic Renal Failure--he never told me what is is. Can you explain?

When I go to see him in 2 months, do you know what are the proper questions to ask?

Should I ask him about my sugar? Would he know of my hpertension?  What does it mean on the Order I take every 3months for blood, it says Creatinine Clearance--HT and WT?  Now what numbers should this read so I get an idea of how I am doing?

Also, under COMPREHENSAIVE METABOLIC PANEL it has: Basic Plus  ALT; AAlkaline Phos, AST; and it has Albumim. Total Protein. --what do these mean and is there a normal readin for it? Also, he has marked GLYCOHEMOGLOBIN.  WHat should this read? He has al checked CBC (Aytomated diff) what does this mean and does it have a normal reading? The last time I saw him he starteds giving me VitaminD--does this mean anything?

Doctor. sense this is new to me, I would like to know what is going on.  I have Diabetes 2, Hypertension.  I ask God to bless you if you will please answer these questions and let me hear from you.  Thank you and GOd bless you.
