Q: Visual migraine without headache leading to feeling very strange for two weeks at a time afterward. What's going on here?

I have my first Ocular migraine 9 months ago. The usual colored spikes jiggling and spinning with blind spot in the center, left eye for 40 minutes. Felt fine afterward but slowly started feeling detached, fatigued, drugged, mentally foggy, and like I was sort of mildly brain damaged. These symptoms continued for more than 3 weeks then I slowly started recovering over the next few days. Felt great since then until 10 days ago when I had another episode. This time the duration was only 20 minutes and much less pronounce. Once again I felt fine for hours after this happened then slowly began to feel really strange, excessively tired, dizzy, spacey and foggy, mildly brain damaged feeling. For the past 10 days now I have felt this way. Nothing seems to help. Not aroebics, steam, sauna, vitamins, Tai-chi, yoga, plenty of sleep, breathing exercises, etc. Nothing even seems to phase it. No physical symptoms like numbness, tingling, and no headaches or generally short mild ones. Any idea what's going on here? I keep reading that visual migraines without headache can't cause brain damage, but what about a chemical embalance or something? This sucks. Can't work, can't drive very well, don't want to return phone calls, or emails. I basically feel like I'm not all there. It seriously sucks. Is this unusual?

Migraine without the headache is called "silent" or "acephalgic." You can read more about that in Acephalgic or Silent Migraine - The Basics.

 

The problem here is that nobody can tell you via the Internet if that's what you've been experiencing. Tthe only person who can safely do that is a doctor who can review your and your family's medical history, discuss your symptoms with you, and conduct a complete examination.

 

If it is Migraine, then it needs to be treated pretty much like any other Migraine. aerobics, steam, sauna, and the other things you listed can't stop a Migraine. In fact, some of them would tend to make a Migraine worse.

 

Something else I should mention is the importance of getting an accurate diagnosis. To simplify matters and make it easier for people to get information and treatment, doctors usually go by the gold standard for diagnosis, the International Headache Society's International Classification of Headache Disorders, 2nd Edition (ICHD-II). Under ICHD-II, there is no diagnosis of "ocular Migraine." We do see it used, but doctors use it differently, so it really doesn't tell anyone what type of Migraine you actually have. The best thing you can do for yourself right now is to see a doctor to confirm if this is Migraine or not; if so, what type of Migraine it is; and find out the best way to treat it. There are great treatments today, even for Migraines with no headache, so you don't have to just go on feeling like this if it's Migraine. If it's not, the sooner you find out, the sooner you can take care of whatever it is.
