How can I get my insurance company to pay for Botox injections for migraines?

I have been suffering from migraines since I was 12 years old. Since December, I have had migraines that have lasted weeks at a time. The first was 7 weeks, then 3 weeks, then 3 1/2 weeks and so on. No medication has helped to either break the cycle, or keep the migraines away. I have tried just…
