Q: medical 504 letter

My 16 year old daughter has been in on-line school because of her intractable migraines.  She had to miss to much school and could not keep her grades up or attend regularly. 

 

She has been doing better because she is in on-line school and because of regular appointments with the headache centers accupuncturist.  She is also seeing a cranial osteopath that has been helping her.

 

She will be attending the local community college in the fall as a Running Start student.  She had a medical 504 in place at her local high school and I am trying to get one in place at the college.  The college wants a letter from her doctor and the doctor wants to know what needs to be in the letter.  I am not sure how to answer that, I had assumed he/she would know. 

 

What do I do?

I have had to do something like that at my university, all you have to do is call the college and ask them you may get transfered a few times but that is the best way usially submissions of medical documents take alot longer at the the post secondary level.
