IMATCH, Interdisciplinary Method for the Assessment and Treatment of Chronic Headache is a program that combines the medical, physical emotional needs of the patient in addition to physical therapy, counseling and group therapy. 

While I personally have not heard anything about it, there may be other out there who have and will be along shortly to assit you.
