I really want to help you with an answer to your question, but can't, because it cannot be adequately answered online. This is a very personal decision that only your and your doctor can make together after an accurate diagnosis has been made. That can only be provided by a doctor who can review your medical history, your family's medical history, discuss your symptoms, and conduct a complete exam in person.
Since Migraine is a genetic neurological disease thought to be caused by overactive neurons in our brains and genetics, having surgery won't get rid of the disease. It may however, help reduce a trigger - if lower back pain is a Migraine trigger for you.
Usually, from my point of view only, I would try every non-invasive option out there before having surgery. But that is just me......
