I have had the pains that you are speaking about. I wanted to share my thoughts and some of my experience with you. First of all, a neurologist is not necessarily a Migraine specialist. I have seen neurologists that were not helpful AT ALL!! But I have seen Migraine specialists that are extremely helpful. They know more of what your husband is going through. The pain is scary for sure! I have gasped and have been brought to tears when those pains hit. Thankfully, they usually don't last very long. My longest one lasted maybe 15 seconds but it seemed like forever. Has he had any tests done? I ended up having a cerebral angiogram last fall because it was suspected that I had an brain aneurysm. It showed that I did not have an aneurysm. This brought me great peace of mind. Now when the pain hits I know it is part of a Migraine attack and not a stroke. I had had a lot tests that led up to the angiogram. I strongly urge you to find a Migriane specialist. It is worth driving some distance if you need to. They will get your husband the tests he needs and find out if he is at a risk for stroke. One drug that I still take is called Indocin (indomethacin). I was told that this drug is good for ice pick headaches. I can only take it 2 days a week so I don't become at risk for rebound headaches.I am also currently on 4 preventive meds. I just recently had Botox injections for my chronic Migraines.

Treatment depends on what the stabbing pains are. Did the neurologists your husband saw give him a diagnosis?
If they're ice pick headaches, the problem with treatment is that they're so short that they're over before any treatment could work. When ice pick headaches are frequent enough to warrant preventive treatment indomethacin is usually used. You can find more information on ice pick headaches in Ice Pick Headaches - The Basics.
None of the reading I've done indicates that ice pick headaches are related to stroke risk, but this is a very real fear for your husband. He really needs to discuss this with his doctor. If his doctor isn't able to help, it may well be time to consult a Migraine and headache specialist. It's important to note that neurologists aren't necessarily Migraine and headache specialists.
