Q: Five auras in one morning!!! What the heck?

Waiting on my doc to call (all day! not looking good) and need some reassurance since no one I know has been through this. I had 5 auras this morning between 4 a.m. and 10 a.m. Maxalt kept the pain from getting out of hand up until about an hour ago and it's creeping in now but I've had my limit of two pills. I've had two auras in a day, even in rapid succession. But five? Is this common? I'm sure background would help but let's start with the basic question first. I will say, in short, I've had migraines since I was 22 (auras since 27) and am 46. I also have autoimmune disease (Undifferentiated but "lupus-like"). Thank you, KC

 

another note: I had been aura-free for about a week or so. Only recently started diary so no exact date.

Hi KC,

 

By now, I hope you've heard back from your doctor - what did he say?

 

The International Headache Society has a headache classification of headache disorders and has what is called Persistent aura without infarction on their list. This is fairly rare, and may occur when someone has aura symptoms that last for more than one week without radiographic evidence of infarction.

 

It is possible to have multiple aura, but any time  you have a change in your Migraine pattern, or severity or frequency, your doctor needs to be notified.

no - they never called back. I'm seriously disappointed. I suspect the nurse never passed on the call to the doc. I can't imagine my doctor would just ignore this. I'm shocked the nurse seems to have done so.  Clearly I was shaken up and had trouble remembering my own birthday so you'd think she'd have thought it serious enough to at least all and say, "we can't help, go see x" or something!

 

I'm looking for someone in the area who is a specialist in this.  The frequency has increased in general since November. Maybe seasonal. But yesterday was unique. Today no auras but the pain came back. Another Maxalt and the pain abated.

 

I assume it would be unwise for me to try to get my workouts in during this time. Normally the pain would preclude any workout but with the Maxalt the pain is mild. The Maxalt is new to me.

You absolutely need to be persistent with your doctor. I'm not giving you this information to scare you, rather keep you informed, ok? When you get a chance, take a look at Migraine and Stroke Risk.

 

One issue could become a  problem is Medication overuse headache. This can occur if you take Migraine abortive meds such as triptans or any kind of pain med more than two days a week, a big part of your problem may well be medication overuse headache (MOH), aka rebound. Hi Nancy.

 

Yes, I'm familiar with rebounds and have had them.  Don't know how that would apply here but definitely a general increase in these things could be in part due to rebounding.  I only started the maxalt a couple of weeks ago. Prior to that I took a hydrocodone/aspirin cocktail (which does indeed encourage rebounds and doesn't kick the pain like the maxalt did).

 

doing okay today. Rested a lot yesterday which helped.

 

I finally got a call from *a* nurse about noon. Not the nurse I called 2x and the nurse that should have taken care of it. But a different nurse, who bless her heart, hadn't even been given the right information!  I'm not impressed. Makes me want to just up and find a new GP if this is the way things are going to be.  The nurse who I usually dealt with retired not long ago. She was very sensitive to the patients and was always prompt.  She would have gotten in touch with me as soon as she got the message even if it was just to say, "You need to see an expert."

 

So - that's the advice now, three days later.  They've given me a name of a neurologist but now I'm thinking I need to do a little more research on my own.  I just don't know where to start.

 

I also don't know if I can try to get back to my workout (elliptical) or what?  When I'm not in pain that is.  Any thoughts on that?

 

thank you for the input.  I'll keep in touch on when something happens.  I see my rheumatologist in two weeks so it's possible she'll have some input if I can wait that long.  Obviously I'm not going to keel over at this point but I didn't know that three days ago.

Yuck. Sounds a bit reboundish to be (but then again I am no doctor, let's be very clear on that oneSmile) especially seeing as you were taking the "hydrocodone/aspirin cocktail" you mention and then followed it up with Maxalt......anyhoo...

 

Are you still having aura's? Has your pain has lasted over 72 hours? See here's the deal - a single Migraine that lasts 72 hours or longer with less than a solid four-hour pain-free period, while awake, is termed status Migrainous. You should see your doctor or go to the emergency room if a Migraine lasts that long. See Status Migrainous - The Basics for more information.

Well, I've certainly had 3+ day migraines without a 4-hour or more pain-free period.  not in the past week, fortunately.  In November I had 6 days solid of no real relief. The edge would be taken off but it would come right back.  Again, that was probably in part a rebound situation though because I didn't have the triptan.  Since then I can take a Maxalt with an aura, then be free of pain for a full 24 hours or more.  But I've also had that one day that I had the multiple auras. 

 

I'll look at the list.  I was considering looking at Angie's List (is that the name?) in my area as well. 

 

I just can't see going to the ER. My sister has done that - lain in a hard bed for hours on end with an IV and bright lights and noise all around. How does this help?  And I'm not being snitty, I really mean that as a serious question.  Aside from all the UV light that would send my body into serious rebellion with fever, rashes, and joint pain. I was so sick after spending six hours at the ER with a friend who had an allergic reaction.  I just don't think in that situation they can help me as much as the environment would harm me.

Most migraine specialist tell us if we have migraines and they suddenly get worse or more often we should seek medical attention immediatly.

one last night and one again this a.m.  Have an appt. with neurologist on Thursday. She specializes in neurological issues that autoimmune patients deal with including migraine so she should be a "big picture" type.  I met her years ago when she gave a talk at our lupus group meeting. Had forgotten all about her until I saw her name in my insurer's database.  It's a start anyway. At $3 a pill, I can't do the Maxalt thing forever and can't tolerate the lack of communication from the GP. 

 

At the moment, the flashing lights are gone (left side) and I just have a big blur on the right side of my FOV.  What fun!

 

Thanks everyone for your input. Will report back when I find out anything. Even if it's just that she says, "I'm not the doc for this."  (Though her nurse said otherwise).

Hi. I've just read through this. Maybe it's time to check into a Migraine specialist. It's important to note that neurologists aren't necessarily Migraine and headache specialists. I'm really not sure what to say about exercise. If this is "just" Migraine aura, it would probably be safe. On the other hand, one reason we always say to see your doctor if your symptoms change is that you need to rule out other causes of your symptoms.

 

I'm really sorry you're having such a rough time. Will you please keep us posted?

(nod) I haven't written off a "migraine specialist" - my thougths with regard to this particular neuro is that I am flaring in general right now and if she feels it could be autoimmune-triggered than we need to look at that as well and rule out CNS impacts of the illness.  And if anyone knows a good migraine specialist around here, it will be her.  A search on that topic did yield her name but of course, such things can be deceptive. I mentioned migraines to the nurse and she said,  "oh yeah. She can do that."  But we'll see.  Right now I just need to get *someone* who isn't just going to throw their pet med at me (of which they just happen to have plenty of samples). Yell

KC,

 

I have suffered from migraine with aura for 30 years and on occasion have had several auras in one day. I know how scary it is for you. Try not to panic when you feel one coming on, I think it makes it worse. Several of my migraine friends also have had episodes of multiple auras. Hope your feeling better.
