Q: I'm taking 100mg Topamax at bedtime. How should I titrate to 200mg?

I am currently on 100mg Topamax daily taken at bedtime.  My Dr had me titrate up to this dosage, but never gave instructions to split the dosage between am and pm.  She has now asked me to increase to 200mg and said to titrate up again.  This time she mentioned working towards 100mg in the morning and 100mg at bedtime.  Should I just start adding 25mg in the morning week by week until I reach 100mg in the morning to go with the 100mg I'm already taking at night?  Or is this supposed to be more balanced?

Holly,

 

Topamax should be started at 25 mg per day, adding 25 mg per week until reaching the desiere dosage. From the second week, it should be taken in split doses -- i.e. 25 mg morning, 25 mg evening. This is for two reasons -- to reduce the chances of side effects and to keep the blood level steady to afford the best chance of it working well.

 

Given how your doctor had you start, this is a question you need to ask her. For any of us to attempt to answer it would be giving medical advice, which we can't do here.

ASK YOUR DOCTOR.  You (and/or your insurance) pay your doctor a lot of money.  Pick up the phone and nag until she or another qualified health professional answers.

i was told 2 take 25mg twice a day and increase every 3-4 days
