Q: 11 year old started on topamax at 150mg first day and 100mg thereafter...

my 11 year old was started on topamax at 150mg divided into 3 doses and then she got 100mg a day x4 days and then due to so many side effects we began to wean with a 50mg dose on day 6 and then md stopped it all together. she also had phenergan and ativan, some morphine and g-don?, solumedrol, versed, valuim, and chloral hyrated in the mix of all of this, but her nausea and vomiting was extremely bad the day after she started the topamax and then it was after the ativan 3 mg dose that she began to have nightmares and became uncoordinated, from then on had blurry vision and dizziness, plus anxiety, hyperventalating and passing out, 3 days later had an abg that showed a low co2. still shaking now which is 5 days since last dose of topamax. still dizzy and blurred vision and unsteady gait.  still with headach, but on indomethacin. mds wanted to think that headache and anxiety(something wrong at home?.....) was the cause to all of this. did say- well  she did get alot of meds and they do have side effects.... no one wanted to admit the topamax was started too high- which is what i believe and that she had actual oversosong going on. what do you think.

I am so sorry your child is going through this!

 

If your child was started at 150mg - then that is about 125mgs too high in most cases to start out on Topamax. See this article for more understanding: Dosing and Tapering Topamax for Migraine Prevention.

 

The first thing I would do is find a new doctor, to be quite honest. We have a great list of Patient Recommended Migraine Specialists you can look through and see if someone is close by.

Rosemary,

 

I've been on Topamax since 1 June, and was started at 25mg per night for 7 days, then 50mg per night for the next 7 days, then 75mg per night for the next 7 days, and then finally 100mg.  I'm now on 100 mg in the AM and 100 mg in the PM.  My neurologist told me that we had to gradually introduce me to the drug. In my neuro's opinion 250mg or higher in an adult is not safe to prescribe, so I would assume children even more so would have more of an adverse reaction.  I'm surprised the pharmacy would dispense it.  That's a very high dosage. 

 

I would definately look into finding another doctor.  Hope she's feeling better.
