Q: What good is it to go to the hospital for having a severe migraine if you have chronic migraines?

  I had a migraine for 6 days.  I went to the ER and told them that it was on it's 6th day and that my eyesight had decreased and that I was extremely nauseaus.  The pain level jumped between 6 and 10 over those six days.  A doctor came in, didn't even bother to look in my eyes or have me follow his finger.  He just said, "I see you get migraines, I know just the thing"and left the room.  He ordered I/V Benedryl and Compazine.  1/2 hour later he asked how my migraine was.  I said it might be a tad better.  He said fine, then I could go home.  Oh, and I should call my doctor if it didn't get better in 2 more days.

 

With my chart stating that I get migraines, does it make any sense at all to go to the hospital?  Should I go to one I've never been to before and lie to be taken seriously?  How on earth would I ever know if I have something seriously wrong?

Let me ask you a question -- Since you'd had this Migraine for six days, you would have had it on at least four weekdays. Why did you go to the ER instead of calling your doctor? It's always best to see our own doctors if possible. That said, it's also advisable to discuss with our doctors, in advance, what we should do if we need assistance after hours, on the weekend, and when he or she is on vacation.

 

You're right that the doctor should have done a neurological examination before giving you any meds, and he should have tried something else when the Benadryl and Compazine didn't break the Migraine.

 

If the Migraine had been a solid Migraine without at least a four-hour pain-free period, while awake, it's deemed Status Migrainous, and needs to be broken. There are a number of treatments that can do the job, including a simple, safe IV infusion of plain old magnesium sulfate.

 

I wish there were a good answer to give you. You NEED to get treatment under such circumstances, but ER treatment can be a problem. Six days into a Migraine, you need to be sure that's all it is -- rule out stroke, etc.

 

You can try the other hospital, but you don't really know how they'd handle your case. I would suggest calling the hospital and asking for a meeting with the hospital administrator to discuss the problem.
