MissionPoint and Second Harvest Food Bank Partner to Improve Lives through Healthier Meals
A Little Help From Friends at MissionPoint and the Second Harvest Food Bank of Middle Tennessee
Kimberly Harris is grateful. She suffered from obesity, high blood pressure, lupus and fibromyalgia. She had gastric sleeve surgery to help lose weight and was told afterward to eat healthier and gain understanding about proper food portions. She tried to follow the advice, but her budget made buying fresh fruit and vegetables as well as preparing healthy meals a stressful affair. She was at odds about what to do.
Based on the recommendation of one of her doctors, Kimberly joined a pilot study being done by MissionPoint and Second Harvest Food Bank of Middle Tennessee. The groups were working with one another to deliver meals to qualified MissionPoint members. Qualification included illnesses such as congestive heart failure, high blood pressure and diabetes.
Workers from MissionPoint deliver seven frozen meals one time per week for a period of eight weeks. Second Harvest has a facility where it prepares, freezes, and distributes meals that are made from fresh ingredients. The two groups also provide one meal per day to immediate family members of study participants.
Harris defines MissionPoint as a "blessing" and has lost thirty pounds since gastric sleeve surgery.
MissionPoint is an Accountable Care Organization (ACO) and manages health care needs for large populations. The goals of the organization are to improve health outcomes for individuals, lower health care costs, improve satisfaction with the health system, and improve the satisfaction of providers.
The organization is based in Nashville and has more than 100,000 members along with a network of over 1,600 physicians and nine hospitals. Members come to MissionPoint via a health insurance exchange established through the Affordable Care Act.
Members are entitled to benefits from a Heath Partner Model, the Care-at-a-Distance Program, a primary care provider designation, and member services and support.
The Health Partners Model helps members navigate the healthcare system while the Care At-a-Distance program uses technology to help members access speciality healthcare that is closer to here they work and live.
Primary care provider designation allows members to select a primary care provider from the MissionPoint network and member services and support helps patients understand benefits and billing.
The Second Harvest Food Bank of Middle Tennessee
The mission of the Second Harvest Food Bank of Middle Tennessee is to feed hungry people and assist in solving hunger issues within the community. About 600,000 people throughout Tennessee receive food from Second Harvest each year.
Food is distributed through non-profit agencies such as low-income day care centers and food pantries. There is an Emergency Food Box program geared to help individuals and families that are having emergency food needs.
Second Harvest also manages 17 Kids Cafe® sites where at least one hot meal per week is provided to children in the communities poorest neighborhoods as well as 42 BackPack sites where weekend food is provided on Fridays for children who are at risk for hunger.
