If you are morbidly obese or have a BMI over 40, you should be able to petition your insurance company (with your doctor as advocate) for coverage of this surgery, especially if you have strong risk factors for disease or clear disease present.    If you have no coverage, you might present to some university teaching hospitals whose surgical residents need surgical practice (the attendings will monitor the surgery) but you will still have to pay for hospital stay and anesthesia and any other outside costs.
