Q: I have a new patient who states she has taken either fosamax or boniva for the last ten years and the test results have not changed from ostepenia, suggestions? Labs? Actonel? support? thanks jane

Is there a recommendation for a drug holiday? Besides calcium and Vit d, any other testing? jane

Maybe the t-score (-1 to -2.5) is normal for her? Remember, the classification of osteopenia and osteoporosis is really just a statistical line along a bell-shaped curve with some individuals 1,2, or even three standard deviations away from the average. The T-score is just a guess at what is "abnormal" based on what is normally expected. In reality, her bone density may be normal for her. Statistics do not represent reality. And less dense does not neccessarily mean more fragile. Take balsa wood for example. Balsa wood is a very light weight, less dense wood, yet is structurally very strong which is why it is so flexible, not fragile.

Pound for pound (balsa wood) is stronger in some respects than pine, hickory, or even oak.

 

The balsa wood example is helpful to keep bone density in prespective.

 

The real question is: does she need treatment at all? Does she have a high fracture risk? What is her FRAX score? Does she have a history of fractures? What are the goals for treating a number anyway?

 

Exercise, vitamin D, calcium and a little reassurance might be all that the doctor needs to order depending on the rest of the story.
