Q: My mom has very low phosphate in blood-0,3, normal Ca levels,PTH-125 ng, AKP-3x high than normal.Muscule cramps,weakness,pain in the ribs. Bone scan show biger aptake of Tc99 in bones.What is therapy?

The first question that needs to be answered is: What is the diagnosis? Top on the list when faced with a hot bone scan (T-99) and elevated bone alkaline phosphatase (AKP) is metastatic lesions in the bone from malignancy. The combination of AKP and a bone scan increases the sensitivity and specificity of detecting metastases. With an elevated parathyroid hormone level (PTH), the chances of having lung, kidney, pancreatic, or ovarian cancer are high. Lung and kidney cancers are well known to metastasize to the bone.

 

Well, I am afraid that none of this is good news. But, establishing a correct diagnosis and ruling out the worst case scenario is critical.Treatment cannot be determined otherwise.

 

Your mother needs to see an oncologist (cancer specialist) as soon as possible for a full evaluation.

 

In this case, I hope I am wrong.

My dear Collegue.This is situation wich we have more than 4 year with my mom.It seems that there is Osteomalacia in progress. All other results are OK,Sedimentation,Tu.markers and all.She has low Phosphates,high ALP,normal Ca and double PTH.We have sugesstion to try to treat her with Vitamin D, Phosphates and Ca.Do you have some experience with this desease? If you have write me how to dosage this supllements? Thanks a lot
