Q: DEXA scan reports osteopenia, but hip surgery reveals osteoporosis

My 2008 DEXA scan showed the bone density in my hips to be in the osteopenia range, yet I recently had my left hip replaced due to osteoarthritis and my surgeon said that my bones were very thin. Due to their thinness, he had to put in extra pins and a plate to attach the implant to the bone. I had been taking Fosamax for 3 years prior to this surgery. Now, my gynecologist is suggesting that I start on Forteo, which I am very reluctant to do. I have never had any fractures. What would you advise and is there a specialist that I could see for another opinion?

Hi Tea Lassie, welcome to the community.  You don't mention your t-scores so I'm wondering if there are at a point where your Dr. might recommend Forteo.  Usually Forteo is used by those with a previous fracture or t-scores below -3.0.  However this decision is up to the Dr. and some will recommend it if you have other medical issues with your bone and bone loss.

 

If you wanted to get a second opinion, you could see either a Endocrinologist or Rheumatologist, both of these Drs treat osteopenia/osteoporosis.  Since you have a Gynecologist and Orthopedic already they are also considered specialists in this field.

 

I took Forteo and did really well, but I had low -3's t-scores and a previous fractures.  If you aren't sure what to do, I think a second opinion is a great place to start.  See if you can't see a Dr in either of the fields mentioned above and see what they have to say about the prospects of Forteo.  Forteo is not covered by all insurances, and it is very expensive, so it's important to do your homework before considering it.
