Tolerance to meds

Can someone please help? I have been diagnosed with Lupus SLE, Fibromyalgia, IBS, MVP, among other things. I have been taking Lortab 10/500 swapped with Norco 10/325 6-8 times a day for over 4 years. I have developed a tolerance and my pain is only getting worse. Does anyone have any suggestions on new meds I could talk to my doctor about? I don't know what to do and I am at my wits end. I have 3 children to take care of and without painkillers...
