Radical Surgery to Treat Prostatitis

I was recently contacted by another urologist who asked me a very interesting question - Are you aware of using a radical prostatectomy to treat prostatitis? Prostatitis is an infection or inflammation of the prostate gland. There are different grades of prostatitis. Acute bacterial prostatitis dev...

I am a 33-yr old man sufferring from severe chronical prostatitis that started as acute bacterial prostatitis 9 years ago. I have gone thru countless bottles of various antibiotics, some of which do relieve the pain temporarily but the pain always returns with a vengeance.

Prostatitis is an extremely debilitatiting disease, especially during the flare-ups which can last for months at a time. Nothing except strong opiates relieves the pain. I am warming up to the idea of radical surgery.

I wish there were more testimonials.

You would the youngest prostatitis I have ever seen. I would look into working you up for interstitial cystitis. Radical surgery is your very, very, very last option at your age. Also, try accupuncture, osteopathic manipulation, yogam etc.

Good luck

Youngest prostatitis? I don't think so. Even my urologist says he sees dozens of patients in their 20's. We're not talking about BPH here, but prostatitis - inflammation of the prostate - a common aftermath of STDs like gonorrhea, which is exactly my case.

With so many drug-resistant strains out there, gonorrhea travels up the urethra, antibiotics or not, reaches the prostate and has a party. Once harbored in there, it's nearly impossible to get rid of as it forms calcifications which antibiotics can't penetrate and no drugs known to man can dissolve.

After 8 years of suffering, having failed all treatments, radical surgery isn't the very very very last option anymore, it seems like the only one left.

You are obviously very bothered by your situation. Consider talking with Dr. Korngrad in Florida. He has a good website.

Just like you im 33 years old, i had been suffering this  nightmare for almost two years, but every single urologist is underestimating the pain, the frustration and the progressive  lost of quality in your life...i mean YOUR OWN and UNIQUE LIFE !! i cannot imagine how you can resist 8 years. Im already in contact with the doctor  Krongrad because the last option in our case could be the only one, this is the only testimony that i know about radical prostatoctemy for chronic prostatitis, and after a really intensive search i can tell you that is not so much scientific evidence about the issue . I hope help you, help us, the Dr is a really nice person ready to listen an help; take a look.  Best Regards. German

I am 26 years old battling chronic non-bacterial prostatitis.  I have been going through this for almost a year.  Been off work for a total of 1 month and light-duty since april 2008.  This has taken control of my life.  If you have never delt with this then how can you say that someone is too young or there are other options.  I have been tested for interstial cystitis 3 times because I was told I was young.  Well the medical field relating to the body works in mysterious ways and this is one of them.  I am absolutely behin dhaving the surgery.  I would do it today.  The anti-inflammatories don't work, pain medications don't help, flomax doesn't help.  My stream is still weak, very hard to start, nothing works.  I think that if someone wants this done then it should be done.  I know the risks and would do it today.

I'm 32 yr old man who's been going through the same thing for 10 fricking years! I have tried lots of medicine and antibiotics. The doctors have no clue ! I have to share with you guys the things that helped me most : There is this broccoli cure from a Turkish doctor. If antibiotics or herbal supplements like saw palmetto helped me 10 %, I would say this cure helped me 80%. Basically you get 200-400 grams of broccoli ( better organic) Put it in a liter of boiling water. Boil for 5 minutes in light heat. Drink the half in the morning before you eat anything. Drink the rest in the evening before dinner. Do not eat anything for 20 minutes after drinking the water in the morning and evening. You do this for 7days then stop for 3 days and start over. You can do this a minimum of 3 weeks or up to 90 days depending on how bad the condition is. You can eat the actual broccoli in your salad etc. but you don't have to. This will ease the flow greatly. You will feel the difference in only a few days.

His other tips : Do not eat spicy food. Do not drink soft drinks. Do not eat or drink something cold. No caffeine. No weight lifting or carrying heavy items as this increases the pain. He also reccommends consuming white onion. One thing else that I find really helpfull is drinking lots of fresh squeezed veg. juice. Eat as healthy as you can. I'm sure this will help.

Suffering from more than 6 years - I did try almost every kind of treatments, several brands of antibiotics, pain killer, AINS - acupuncture and lately 10 runs of  lithotricy. No results at all , except some pain relief. Since a month, I do take fytosil - a silicium based water with horsetail and stinging nettle - which does help to maintain pain below acceptable level.

No need to go to the bathroom during the night, sensation of a 'brick' between thighs slowly disappears.

Think we are on a good way.

I am going to see a Dr. if he would remve my prostrate, in hope that would cure my prostatitis. I have been in pain for the pass 9 years and it has effected my quality of life, work, marriage and my relationship with my kids. Gary, Patrick ,any one who gonna makes the surgery or just made it please let us how is working, over all the evolution post- surgery; its more than important creates a bank of experiences that help us to fight with this frustrating illness, chronic prostatitis.

Actually im living in Poland and two weeks ago i had the opportunity to talk with one of the bests urological surgeons in the country -extremely expensive of course- and  told me that in my age (33) and my case (almost 2 years with nonbacterial chronic prostatitis) the surgery is not an option, because can affects my urine control and my sexual strenght; even he gaves me statistics: 35% of the cases have troubles with erections, and 25-30% with urine control... at least in Poland.

Then whe i asked for a solution or somekind of relief in my conditton (i cannot sit) he told me.. patience, patience, you are young and strong enough...?!?!

and prescribes me IBUPROM MAX, some kind of analgesics like Tylenol, and some suppositories, thats all.

The point is that he scare me about the surgery but not convinced me at all; i cannot imagine living the rest of my life with this and personally is a solution to considerates.. i still didnt make it by economical reasons, but talking with doctor Kongrad i feel myself a little more confident, even when he tolds me that doesnt exist serious researchs about the issue, ergo no much scientific evidences that support it, i mean the radical prostatoctemy in chronic prostatitis.

This is another link to check about similar experiences:

http://chronicprostatitis.wordpress.com/2008/03/24/surgical-treatment-of-chronic-prostatitis-with-radical-prostatectomy/

Well i would like encourage every man that is suffering with that to enjoy us, to help us with his experience, maybe working together we can found -besides the support- a possible solution; at least we can create a path to follow for others, even for themselves. Waiting for answers.

Good energy for every body.

To the doctor who posted that 33 is the youngest case of prostatitis he ever saw..amazing this is what we must deal with.

This is why we are all so frusterarated. Mosr doctors only know how to take our co pays. Most prostatitis starts in 20s and 30s

dr krongrads forum has great info on prostate removal..

Krongrad did two surgerys. One is a full cure, the other guy is recovering well

dr eden in uk did 5 surgerys for prostatitis. All 5 werwe cured

Dr eden reports 99 percent have urinary controll and if your healthy sexuly and young, the odds are in the mid 90s for sex function to return

its all about surgons skill with this operation.

I am 48 years old with history of seven years chronic prostatitis. On January 29,2009 I had Laparoscopic radical prostatectomy done (LRP) with Dr. Krongrad in Aventura, Florida. My surgery went well. My debilitating pain and weakness is gone although I am still recovering. IF you want to read more about my surgery go to Dr. krongrad web site or prostatitis.ning.com

There is a clinic in arizona that does prostate massage therapy with antibiotic treatment if bacterial prostatitis is the problem. there is also a treatment center in china that is supposed to have a good cure rate but expensive and far. There is another doctor in new york that shoots antibiotics directly into the prostate but i dont know the success rate or saftey of that procedure. It would probably be worth looking into these options if all else has failed.

I feel your pain 100%.  I'm 35 and dealing with this myself.  It started very mild a couple of years ago, would come and go as the months go by, just felt like I had to pee often.  FF to this year, and the symptoms are much worse and I haven't found any long term relief.  Sometimes this is feeling as if there's an object suck in the base of the penis and you can't pee it out, though urinating is not a problem.  Not pain, but certainly discomfort.  I recently had the procedure where they put a camera down your penis to look into your bladder, and when he was right about my prostate with the camera that was the exact feeling that I get (object suck in there).  I tried cipro and uroxitrol (sp?) and got some immediate relief but it was not long lasting and my urine tests come back clean for bacteria.  I get short spurts of feeling great followed by days of complete discomfort.  If I'm feeling great at the moment, then sex can be fantastic, though the next few days can be terribly uncomfortable.  IBProfin relieves my symptoms sometimes, as does refraining from everything sexual for a couple weeks at a time.  Anyway, lately I've been uncomfortable more than comfortable, and if removing the prostate can be a cure, then that's certainly something I would entertain.  For someone who has never dealt with this, it can really wreck your life.  Can't sleep, can't concentrate, iritable all the time, relations with your wife certainly suffer, etc..  So you end up with no carreer, no wife, no drive to do anything and now ay to get rid of this discomfort.  It really wrecks your quality of life, and if there's no other likely solution and removing the prostate would fix it then sign me up.

It's now 1/23/2010 and I'm following up.  As I'm getting older this is becoming more persistent (been dealing with this on and off for several years now), though I'm learning how to control the symptoms a bit.  For me sex is still great fun if my symptoms are under control at the time of sex, though I'm frequently very uncomfortable for several days afterward.  IBProfin can help.  Also my doctor recently prescribed me Phenazapyridine 200mg to take as-needed, and this has helped take the edge off the symptoms greatly.  I sometimes actually feel normal after taking this.  Intense exercise and lots of sleep also help.  I'm frustrated because the more sex I have the worse the symptoms get for me.  I'm learning to deal with this, but I am trying to research the risk vs. reward of having your prostate removed for the purpose of perminantly eliminating these symptoms.

I am 24 years old diagnosed with non bacterial prostatitis. I have had this problem since i was 20 years old. I have taken any medication you can think of to control my symptoms. I have tried everything from massages to acumpucture to exercise, all these things are relief only temporarily. I am in great shape but have a problem that no one can treat. Living in Los Angeles I have tried to take advantage of the best doctors money can buy, and still nothing. Anywhere from St. Josephs, to USC to UCLA to Seider. Now I am at the point of considering the various lasic surgeries to help with this. Has anyone considered that as an option? I do not want to remove my prostate at such a young age, but am willing to try other surgical treatments to resolve my discomfort on a daily basis.

12/ 5/10 4:19am

I had the surgery, and I had mixed results. I had the surgery about six months ago. I do still have the same pain that I had before the surgery, sorry to say. But it is true that the pain is somewhat less intense. I still have to take pain medication at the same dosage and strength as before. Somehow the pain is not quite so absolutely terrible, more bearable. But still present, most definitely. I hope this helps. This is absolutely true. I think that Dr. Krongrad is coming from a good place, trying to help men, but this surgery does not always work. There could be other causative factors, such as pudendal nerve entrapment, trigger points, myofascial disorder, even stress aggravating things (not causing the problem, though). Anyhow, be careful before you have the surgery, though it may work for you.

FrownDear Dr.  My husband has has this terrible problem for 6 years to no avail. We are now looking into surgery and hope we can find someone who will perform it. We were told by our family Dr. that back in the 50's/ 60's this surgery was done numerous times because  antibodics didn't work and I can atest to that. It has been a long terrible 6 years.  You may want to check out The   Krongrad Institute in Miami where this has been very successful.

I am a 49 year old male and have suffered with prosttitis since my early 20s. I have taken many antibiotics over the years and cipro and levoquin work best for me. The antibiotics help but the infection always comes back and my symptoms have worsened over the years. I recently had a ct scan and cystoscope that showed an enlarged prostate and bladder thickening. This disease has affected the quality of my life to the point that I have not been able to work in over 2 years. I feel as though I have the flu all the time and have little stamina. When I was younger I was very strong and could work almost endlessly. I have already considered the surgery and contacted Dr Krongrad but cannot afford it at this time.

I am a fifty-two year old male who has been suffering from non-bacterial prostatitis since 2004.  I have all the classic symptoms as previously described by the others who have left comments on this page.  Through prostate massages and hot soaks, I have been able to control the symptoms up to three months at a time.  However, it always comes back, in spite of my efforts to prevent it from happening.  The symptoms seem to be harder than  before to get under control.  Although my urologist seems optimistic, I am becoming increasingly frustrated and pessimistic. I want a permanent solution to my chronic pain.  Someone please tell me what to do.

12/30/10 1:27am

I've had the prostatitis and enlargement with terrible pain for 14 years. In January of this year I considered prostate removal as the solution.  I asked the doctor to try one more drug, Proscar (Finasteride - 5mg/per day).  After 10 months of taking it, the pain is gone, my PSA is lower, well below 1. My prostate is smaller, stream is back to normal and I feel better than I have in years. It did lower my sex drive a little, but I have no problem with erections.  I would give it a try and wait for results, as it took me a little longer to start seeing a change in my condition.

JP

1/ 5/11 1:43pm

Thank you so much for your reply.  I will see my urologist tomorrow and ask him to to prescribe Proscar for me.  This information is right on time, because I am going through a flair up right now.

1/ 5/11 6:53pm

Tracy,

Your welcome, another great thing about Proscar is while it was shrinking my prostate, it reversed the slight thinning of my hair. It grows thicker now. The urologist did tell me that this drug may cause impotence in a small pecentage of men that take it. I have low blood pressure, so the Avodart and other similar drugs were off limits to me because they lower the blood pressure. I thought it was worth the small risk of sexual side effects, if it did away with DHT that causes all the prostate problems.  Good luck and I'm glad I stayed with it long enough to see good results.

11/ 8/10 11:59am

I'am a 37 year old male who's had this condition for six months.  This may not seem like a long time, however when you first think its an STD your devastated.  Then you find out its not.  Well little comfort.  My urologist felt he could do nothing more after 2 bouts of anti-biotics.  All the testing he did came back negative.  I'am forced to suffer in silence as my wife and other people don't understand.  My brother is a Doctor and he can't help.

My symptoms include sore testicles, like I was kicked.  Also, the head of my penis is sore and is red (swollen) around the opening.  Reading, this seems to be a rare condition.  In the middle of the night  I get very sore behind my pubic bone area.  When I sit up out of bed it akes.

I wish there was more reaserch with this problem.  I have always been very healthy except with this.

2/ 3/11 3:06pm

I had prostatitis for 20 years (since I was 13), then it went into remission for 6 years, then came back. After it had been back for two years, my life had become unbearable. Having tasted pain-free living during the period when it was in remission, I knew how good life could be without pain.

I tried everything I thought had a reasonable chance of working. finally, I found a surgeon who performs radical prostatectomies for prostatitis. I had the operation and it helped me immensely. I am three months out now, and my prostate pain is completely gone. My continence is almost fully returned, but I still squirt a little bit. I have had a couple of semi-erections, but erectile function is still not there. Apparently, that can take a bit more than three months to return.

So, i would say that if your life is just overwhelmed by prostate pain, a prostatectomy might be a good thing to explore.

2/ 3/11 3:15pm

A follow up to my previous post.

I reached the conclusion that prostatectomy might work for me after I underwent intraprostatic injections. These injections included steroids, an anesthetic, and antibiotics.

The injections helped some, but their efficacy decreased over time. However, one thing I definitely noticed was that the pain was almost completely gone during the first few hours immediately following an injection - this would be the period when the anesthetic was in effect. My urologist (not the same doctor as the doctor who removed my prostate) and I discussed this, and we reached the conclusion that, if numbing the prostate removed the pain, it might make sense that removing the prostate would have the same effect.

Just an idea to explore for those of you considering removal, as I did.

2/ 9/11 2:03am

Ok, we talk about radical prostatectomy. However, I think, should consider the age and consequences to young males. How does radical prostatectomy the sexual life and fertility (in case one would like to have babies?)

2/ 9/11 10:43am

I think radical prostatectomy should be viewed as a treatment of last resort. There are other treatments that can help.

Sitting on an ice bag, for example, can do wonders for pain control. Ongoing opioid maintenance may be an option. An implanted pain pump might help - it worked for me for a long time, until I had a really bad adverse reaction to the medicine in the pain pump. After that, opioids were no longer an option for me for long term treatment.

Radical prostatectomy absolutely makes a man sterile. In fact, after a radical prostatectomy, the patient will not ejaculate at all. He will have dry orgasms from then on. I believe there are some IVF methods that could be used to father a child, but that is expensive and not necessarily reliable.

But, chronic prostatitis can be just horrible. A prostatectomy might not make sense for a young man with moderate pain - but for someone who was at his wit's end, like me, it was appropriate.

I am 44 and have had Chronic Prostatitis for about 8 months. Been on Cipro, Lavaquin, sulfa drugs - all with no lasting effect. I went to Dr Bahn and had 4 prostatic injections about 10 days apart. The injections are uncomfortable but quick. The pain afterward for me was excruciating enough that I took 2 oxycodones to sit down in my car. For the next 3 days, I was drug free but the symptoms slowly crept back. Since then I have seen Dr Curtis Nickel for consultation and EPS testing. I had a white blood cell count of 30 in my microscopic slide. Dr Nickel said this could be a result of the inflammation but who knows. He is a very knowledgable Canadian doctor who has studied prostatitis for a lot of years. That being said, no one has a cure and as I understand everyone has differing results with all of the different treatments out there. This is why urologists get frustrated and patients even more so. Personally, I am now at the point of pain management with NSAIDS and OxyContin pretty much daily. If I go without drugs for 1 day (not often), I consider it a success. As I hate the idea of living with this disease and the underlying suspicion that it will lead to BPH or, even remotely, cancer, I think my end result will be prostatectomy. As I say to my wife, and I am sure most of you will agree, I would rather piss in a bag and have a semi-hardon than live with this pain for the next 40 years. By the way, my wife agrees with me. Now if we could only get urologists and mainstream medical on track to understanding the debilitating pain we live with and how much we want and deserve a cure. This disease should be ranked up there with Arthritis and Diabetes...instead of being continually brushed under the rug by uncaring or ignorant physicians. Good luck to all prostatitis sufferers and continue to push and pressure your urologists for solutions, whether pharmaceutical or surgical.
