Question about irritation of glans - red

I am a 43 year old male. I had unprotected sex in April. STD tests negative, but did regimen of doxy as a precaution. The tip of my penis is red and irritated (at the opening with ever so slight intermitant pain) and I have some incontinence. It is not painful to urinate.

I saw a urologist who put me on cipro regimen for a few weeks. This did nothing. He then indicated possible inflammation, and did an ibuprofen course. The symptoms remain.

I also sometimes have dull pain in perineum. I asked for PSA test -- Urologist said it was not warranted. I am a cyclist and do about 50 miles/week as well. Something has definitely changed "down there".

Any ideas on what this could be - and next course of action?
