Urethra irritation - tip of penis (opening) is red

I am a bicycle rider and recently began having issues. The very opening of my penis is red and irritated. I also have some incontinence all of a sudden. My urologist did cipro - then ibuprofen 800mg - neither of which changed symptoms. There has definitely been a change. The next step is a scope. Should I do this? What could cause these symptoms?
