Elevated PSA in a 45 year-old and better ways of detecting cancer early.

By kc46 Wednesday, April 18, 2007
I am 46 and 8 months ago I had my first PSA of 5.6. Biopsies were negative (8 samples), ultasound estimate of 35 grams. I am worried that if this is cancer this young (caucasian, negative family history) it may be very aggressive and metastasize early. Is there a better way to be sure than annual biopsies (needle in the haystack)? Further complicating this is that I took Avodart for 7 months to reduce PSA and mild symptoms and PSA only dropped to 4.8. Since Avodart usually reduces PSA by 50% has my PSA really nearly doubled to 9.6? It will take 6 months to leave my system to find out. Please help, I can hardly sleep at night due to worrying about this!

I can understand your anxiety. Personally, I would not treat your PSA with medications unless you have urinary symptoms of BPH. At your age, you would do better with Uroxatral.

There are no other ways to detect prostate cancer besides the biopsy. You may have to get another in the future. You should get a lidocaine block and have at least 12 cores taken.

My PSA elevated to about the same levels at 51, my biopsy showed no cancerous cells, I waited 9 years, through 4 more biopsies before any cancer showed in a biopsy. I ended up having surgery at 60 years old, it was sucessful, I am very healthy and do anything I want to, but the cancer had involved enough of my prostate that to remove it completely, damaged my nerves. My advise is to have any treatment you chose early on, the chances of nerve damage increase the longer you wait.  My urologist did not tell me this, I was only told that the cancer was very slow growing and that I had time, true for the spread of the cancer, but not so true for chances of nerve damage during treatment to remove the prostate after cancer is found.
