Q: My father is 74 yrs old. He had urine retention and diagonized Septicemia UTI. Now his PSA is 240.

He was administered 1gm Meropenum IV antibiotic for 9 days (Twice in a day with 12 hrs gap).  Still he has temperature (102 deg. F).  He is very weak and unable to move.  Blood & urine culture reports shows no growth after 48 hours.   His proatate is hard & enlarged. Please sugget.

If the diagnosis is truly septicemia from a UTI, than the PSA is completely inaccurate at the present time. After complete resolution of the infection, the PSA should be reevaluated. If there is a downwards trend but the PSA has not yet normalized, further time and repeat testing should be allowed prior to considering any prostate biopsy.
A hard and enlarged prostate may be significant with regards to prostate cancer but if the infection is arising from the urinary tract, the examination needs to be repeated after resolution of the problem.


The resolution of the septicemia and the UTI is most important. With time, the proper antibiotics, and the proper drainage of urine from the urinary tract, the infection should resolve.
