Can there be cancer in the prostate if the PSA has remained constant at 7.1 for one year in a 69 yr

No other symptoms except prostate slightly enlarged (taking saw palmetto for 10 years). Have been advised to have a biopsy. Can these release cancer cells into the system? Do I really need a biopsy?
