The PSA should not rise after radical prostatectomy. The fact that the PSA is rising implies that there may be evidence of a local recurrence and this is more likely as a result of the pathology penetrating the capsule of the prostate. However, the rate of rise of the PSA is extraordinarily slow. Consideration can be given to either observation or the use of radiation therapy.
