Q: High PSA (and its velocity) with negative biopsies

I'm almost 65.

My PSA test and biopsy results for the past 3 years:

 

12/23/2005 PSA 8.43

02/01/2006 Biopsy (8 samples) negative

07/21/2006 PSA 8.43

07/07/2007 PSA 15.31

   Took an antibiotic for 10 days

08/21/2007 PSA 16.07

09/20/2007 Biopsy (8 samples) negative

01/02/2008 PSA 18.94

02/13/2008 PSA 20.57

06/03/2008 PSA 22.99

06/12/2008 Biopsy (8 samples) negative

11/14/2008 PSA 29.34

 

All DREs did not find abnormal.

 

Do I need to keep having PSA tests and biopsies?

The rate of change of your PSA (velocity) and the absolute value are very problematic.  There are cases of prostate cancer that are not able to be detected with traditional transrectal biopsies. There are several centers in the country that are using advanced MRI techniques of the prostate to detect these cancers.  Another option would be a transurethral resection of the prostate in order to get to this tissue that is not reached by transrectal biopsy.
