Q: hematospermia 8 months ago with perineal, testicular and back pain. ejaculating pain goes away,

episode of hematospermia  8 months ago with perineal, testicular and back pain. Took ciprofloxacin for 5 weeks  and resolved. Nonetheless, pain came back around 3 weeks afterwards and again took antibiotics. for 2 weeks this time. I did some urine culture and prostatic secretions cultures aprox 2-3 months after  the last dose of antibiotics and came out negative. The pain  ( testicular, back pain, and perineal, comes around every 2 weeks, and resolves after ejaculating ( meaning i ejaculate , pain goes away,  then I wait , and if within aprox 2 weeks  do not ejaculate , the pain comes back again. When in pain, it seems as if urinating seems as if I am ejaculating. The same thing  with doing feces.It feels like  the whole prostate area is about to explode. The probelm has evolved to a  point that the back pain is so intense, even when my bladder is half full , it irritates my prostate and consequently my  sciatic nerve  and lumbar region.  What do you suggest?

This most likely represents prosatatitis. Prostatitis can be difficult to eradicate in some patients, and in others, it can recur numerous times.  If your urine cultures and prostatic secretion cultures are positive, then treatment with an antibiotic would be appropriate.  The elimination of caffeine, alcohol and spicy foods may prove to be beneficial. Prostate massage can also be of some use.  In some severe cases, the use of a short-term steroid may be considered. In others, an alpha blocker can be of benefit. Consultation with a urologist should help you.
