Q: How can PSA be 86?

Prostate removed in 2007? PSA came down to 52, now is 86.  How can number be so high?

If the prostate was removed for cancer (radical prosatectomy) then this represents a recurrence of the prostate cancer.  If it was not a radical operation, and only surgery for prostate enlargement, then the likelihood is that this represents either an infection or an inflammatory condition of the prostate
