I have had lower back pain aswell as pain down my right leg and right testicle.The pain is constant

the pain usually sets in at night while I'm in bed.My right leg gets a dull almost numb feeling,my back feels as if the pain is raidiating from the tail bone.The testicular pain is localized to the right teste and if I probe it a lump develops on the bottom of the teste.This doesnt happen if I check the left one.Im 40 yrs old and un-insured,so would appreciate any advice.
