Help with sore and unresponsive penis

Hello.  I have a question that has been very frustrating for me.  I'm 32 and my wife and I used to have sex like 4 times a week on average.  About 3 months ago it seems i somehow hurt my penis and ever since it hasn't been working properly.  It's sore on the bottom side.  Sometimes it feels kind of numb and tingly.  It doesn't feel like it has a lot of life to it.  We've prob only had sex like 4 times in 3 months becasue of my problem and I haven't been that hard on those occasions.  The times we have it felt like I was about to urinate when I was about to ejaculate.  I have a hard time getting an erection now and when I do sometimes a little what I would think is pre-cum comes out and I lose my erection.  This is extremely frustrating!!  Any help is VERY appreciated.  Thank you.
