What does fluctuating PSA scores mean?

I have BPH.  My PSA readings in three years went from 3.2 to 4.4.  At that point, my tests came in six month intervals.  My next reading was 6.7.  I had a biopsy (12 tissue samples)which came back benign.  My next three readings were 5.1, 5.4 and recently 6.1.  I am not sure what to do next?

 

Also does a biopsy increase the risk of spreading cancer due to the bleeding?
