Q: elevated psa and dre normal

My dad is an Asian Male from India , age 75 years, in good health.  He does have BPH. He got his first ever Psa blood test done about 7 months ago that came out to be 10.94. Then he got the repeat test done after 7 months and got back as 11.05. His prostate size is about 80 cc. Doc says that his PSA   test has been kind of stable  with may be a bit of lab variation. and his DRE test found nothing suspicious or abnormal except his enlarged Enlarged adenoids
Enlarged prostate prostate. Was wondering, what should we do. My dad doesn't want any biopsies done...Does he need one? Can BPH cause the PSA this high with his prostate size. I am so worried as his daughter. Please help.

BPH can be associated with an elevation of a PSA, especially with a prostate as large as 80 grams.  At age 75, many patients will have prostate cancer that is incidentally found. As we get older there is an increased risk of developing prostate cancer. If biopsies were done, there is a very great likelihood of there being malignancy, and then a decision would need to be made whether or not to treat the cancer.  A course of observation may be a reasonable approach if he remains asymptomatic and if the PSA is very slowly rising.
