Q: I'm on two prostate medications & recently have had to urinate 3-5 times overnite. What could cause

I've been on Uroxatral for about two years and Avodart for about four months. However for the last six weeks or so, I have been having a problem with getting up in the middle of the night 3-5 times to urinate. I do not have this issue at all during the day. It's onlywhen sleeping. Additionally, I have been diagnosed by my Urologist as having Varicocele. Are any of these things related? What could be causing my overnight trips to the bathroom?

Varicocele is not related to urinary frequency. In some cases it is associated with male infertility and may need to be treated.

Voiding overnight, nocturia, is often a result of a prostate issue, however it can be a very complex problem to resolve. Often times the prostate can be the source of the problem and treating the prostate can lead to a resolution of the symptoms. The addition of other medications such as an anticholinergic which will help to relax your bladder muscle may prove to be effective.

Other factors however may be causative. The quantity of urine that you are producing overnight may be problematic, especially if your bladder capacity is not very large. The amount of fluid that you are consuming in the late evening hours can also be contributing to this. Elderly patients may experience alterations in the ability of their kidneys to concentrate urine overnight, leading to increased urinary volumes. Various medications can also be associated with nocturia.

There are currently several phase III FDA clinical trials that are ongoing in the country for a new medication that is being used specifically for the treatment of nocturia and you may qualify for one of them.
