RA and Antibiotics

I have recently experienced living with RA without my Remicade treatments for 4 months.  This in March when I had surgery for sleep anea.  My Remicade was stopped about 3-4 weeks before the surgery to allow my immune system to build up before the surgery.  The week before the surgery was like a week in hell.  My RA pain reared it's ugly head and attacked almost every joint in my body.  At this point I didn't know if I could stand not being on the Remicade for another 3-4 weeks after the surgery.  But I went through with it.  I was put on antibiotics during and after the surgery.  (for those of you who don't know what sleep apnea surgery is, this is when an ENT doc removes the uvula, tonsills, and some of the soft palate at the back of your throat to give you a larger airway.  THen an Oral surgeon comes in and cuts the bottom of your chin bone and moves it forward, which then moves the tongue forward since the chin bone is attached to ligaments to your tongue and puts a plate in to hold it together)  All sounds so simple when discussing it in the doctors office but it was a 4.5hr surgery with a 2 day hospital stay. 

 

Getting back to my story I came through the surgery ok but about a month and a half later in April I started develping severe pain when I was chewing anything.  I also noticed a white spot on my gums near the incision.  Off I went to the Oral surgeon(OS) to see what was going on.  He took xrays and checked it and said that one of the screws in the plate in my chin was probably loose and said that if he took out the plate and screws it would get better.  So off I go to surgery again plate, screws and small infection are all taken out.  He then changed my antibiotics and home I went.  Another couple of weeks go by and I notice a white area aound the stiches in my mouth so I go back to the office but my Oral surgeon isn't there so I see one of his associates.  He looks at it and says I haven't been cleaning it well enough(which was a lot of BS).  He send me home and tells me to clean the area better.  That Saturday I am looking at the incision and noticed that the infection is raging in my gums my husband calls the OS and tells him about the new development.  Sunday morning I am in his office he takes one look at it and sends me to the Emergency room.  At this point an infectious control(IC) doctr gets involved and she has them put me on Vancomycin, flagyl and levaquin 3 heavy duty antibiotics.  From the CAT scan they did when I was admitted they found that the infection had infiltrated my chin bone. At this point the OS has to go back in and clean out the infection to save the bone.  Tuesday morning I am back in surgery to have this done.  So after a weeks stay in the hospital and a pic line put in my arm I was sent home to administer the antibiotics on my own.

 

A couple of weeks go by having seen the OS each week for a follow up I notice that one of my bottom teeth in the front is sticking up above the rest of my teeth, I have straight teeth so I knew this wasn't right.  My next follow up the following week the OS has his saff take 2 different xrays the second of which he proceeds to show me that my jaw was cracked.  So another surgery is scheduled for the next day to fix my cracked jaw bone.  This happened from the infections and all the scraping to get it clean, it weakened the bone so much that it cracked.  Braces were put on my lower teeth and a plate in my jaw to hold everything together.  It has been 4 months later after the first surgery I am now recovering from a broken jaw.

  Wow what an ordeal you have been through! I am so sorry you had such a hard time with infection. I do hope you are not having anymore problems from it.

I am not familiar with antibiotic treatment for RA, but I did do a little research and found some information. Thomas McPherson Brown, MD efforts to demonstate the effectiveness of tetracycline therapy were initiated and demonstrated over 40 years ago. From what I read it looked like low doses of antibiotics over a long period of time are sometimes given by some Dr.'s. Once again that is something you would need to discuss with your Rheumatologist. I googled RA and antibiotics and there was quite a bit of information. Hope this helps. Keep us posted!

Hi Kim, thank you for your information.  I also heard from another person on another RA web site who told me that she had been treated with antibiotics for her RA.  Unfortunately, with antibiotics they kill off all the bad bugs but also the good one's that your body needs.  I found that out with getting a yeast infection in my mouth after the 6 weeks of vancomycin.  But I am happy to say everything is going much better the infection seems to be gone and my Oral surgeon with be taking the braces off my teeth next week YEA!  He showed me the last xray of my jaw and you can see where the infection ate away at my bone.  He said it will take a while for the bone to regenerate.  So with all this so far the RA pain is still at bay.  So I am hoping that this will continue for another few weeks since the infectious control doc won't allow me to go back on the Remicade until a month after I've been off the antibiotics. i wonder if there is a connection between the infection in your jaw and the RA. 

One of the reasons there hasn't been a cure found for RA is because it is individualized.  Our immune system is hyperactive and RA attaches it self to our individual proteins produced in infected cells. And you had a serious infection going on. I wonder if the RA had centralized itself to help combat the infection? 

 

Who knows, but it's something to look into.  You could be the missing link to a cure!!

Hi Ronie, I haven't read much about that part of RA but it is very interesting and I will definately read more into it.  That would be something if what I went through would help in some way at least I wouldn't have gone through all this for nothing.  Maybe it may be worth a trip to Boston to talk to an expert on RA about what has happened to me.  Thanks for the information.  Janet

So sorry to hear about your terrible ordeal and the lengthy healing process.  I hope all goes well from here on out!

 

I've been living with RA for nearly 20 years.  It started in my right knee and gradually moved into other joints.  I was reluctant to go back on methotrexate and I didn't want to resort to the injectible meds, so my rheumatologist suggested I try antibiotics.  I did some research on the treatment and decided to try it.  I've been taking minocycline for a few years now and it has sucessfully prevented RA from moving into any other joints.  I consider myself quite active and have very few mobility limitations.  I don't think I would feel this good if I hadn't started the antibiotics and I'm certain other joints would have been invaded.  The only drawback about the treatment is the good bacteria in my digestive system was depleted as well which eventually caused colitis type symptoms.  I've been taking a probiotic (Culturelle) for a few months and it replenishes the good bacteria needed for healty digestion which has resolved the problems.

 

The relief you've had with the need for antibiotics during this healing process is certainly an indicator that you would likely benefit from taking a low dose antibiotic as RA treatment.  It would be worth discussing with your doctor.

I am a dental hygienist and I vaguely remember in one of my continuing education classes that there might be a correlation between antibiotics use and RA symptoms going away.  The antibiotic that she was talking about was in the tetracycline line of drugs.   It was speculative then and I'm sure that there may be some anecdotal evidence to support it but research takes many years if anyone is even out there actively researching it.  I'd be interested in how it turns out for you!!  Thanks for sharing your information,

About 3 years ago I was taken off plaquenil due to eye problems. My Rhem Doc gave me a choice of treatment, Methotrexate or an antibiotic.

Yes, yes, yes, I try to remember to mention this if nothing works for people I hear about. I have talked with people who are on this protocol.
