r a titer 1:640 ccp antibodies greater than 250 what does it mean?

I recently went to a Dr. for pain and tightness in closing and opening my left hand(bending fingers).  Upon the pysical exam nothing was found.  She ordered hand xrays and took 8 viles of blood.  The test results  

showed a mild inflamation at 23, later I got a call that the RA titer came back 1:640 with ccp antibodies  greater than 250 - testing strongly positive-  and was set up to see a rheumetololgist which I will see finally next week - almost a whole month has gone by.  I have more pain in other places at times, elbow, hip knee feet wrist but not always both sides and not everyday.  Some fingers hurt more than others.  I feel feverish and wiped out - night sweats and sometimes really worried.
