The Social Security website offers a section that includes what they call "disability starter kits", which includes information and lists of what you need to help you through the process. As well, there is a website called Disability Secrets with answers to many common questions from people who want to apply for Social Security Disability benefits (click here). It also offers links to advocates who can help you for free and a link where you can find help with a free disability case evaluation (for links to advocates in your state, click here). Additional information about Social Security Disability can be found on the site's Social Security Disability blog.

The Disability Secrets website looks genuine and appears to offer very good information, but I haven't verified its authenticity myself yet. You may therefore want to use it as a reference source and look into it further before you offer too much personal information. You may also want to pop by your local Social Security office for more information. SSD will also be the topic of an upcoming Beginner's Guide to RA.
