Although I had many of these symptoms, I would have to say the most persistent and troubling was thought-blocking. I couldn't say a sentence w/o losing my train of thought entirely. I would be in a conversation and couldn't remember what the other person just said or what I was going to add. My longterm therapist thought this was dissociation, but I think it was thought-blocking. I just couldn't retain a consistent grasp of what was coming into or going out of my mind. This just made life all the more difficult on top of the other symptoms. Plus it left me very frustrated in therapy because I couldn't maintain a consistent level of therapeutic interchange.
