So sorry to hear about your brother's death, and that your other brother is ill. My older sister died 2004 because she wouldn't quit drinking, but she didn't have mental illness. On the other hand, I have sz and am often noncompliant with meds. I try to rationalize my behavior but it is still self-destructive. At least I have a steady disability income and health insurance and a nice house where I live with my 80-yr-old mother. We keep an eye on each other. But you couldn't make your brother's decisions for him, even though he couldn't make good decisions for himself. Maybe you can dedicate one of your films to his memory.
