Q: Why do I have cramp-like pain with orgasms and out of the blue in the middle of the night?

I am a 22 year old female. I have been experiencing pain with orgasm for several months.  Instead of the release that usually comes with an orgasm, it's like my lower abdominal area clenches up.  It lasts for a few hours and feels very much like the cramps I used to have with my period.  The last time, the muscles in my vagina tightened and my husband was unable to even insert a finger. In the last month, I've been getting the same pain in the middle of the night. It wakes me up and makes it hard for me to urinate and sleep. I take birth control continuously and do not menstruate.  I had an ultrasound last year for unrelated problems and everything was fine. What could this be?

I'm baffled by the "lasting several hrs part" since once the orgasm passes, your vaginal canal should relax and unless you are somehow stimulating a large release of hormones (hence the cramping/pain) I have no other explanation.  does the intercourse last a very long time?  In that case you may be experienceing pain and inflammation associated with the duration.

I always refer to a gynecologist for an actual exam when someone presents with a baffling history - even if you did have an exam last year - I'd check it out.

I have been having the same problem. I just started taking Seasonel, the birth control you take 3 months straight and have no periods. I think this might be what's causing it. I think i need to stop taking it because it sucks to have this happen!!

i have been suffering from this for many years. it came on suddenly at 27 and after two years of exploratory it was diagnosed as a few things: chronic pelvic pain caused by chronic myofascial pain. i am on some medication but the best thing so far in my recovery was physical therapy. not just internal physical therapy for the pelvic floor muscles (squeamishly personal but necessary) and physical therapy for my whole body. our muscles are all interconnected and our nervous system is too (like pain in your belly may actually come from your back muscles or trigger points). stress, etc... may exacerbate this condition and i know that slowly i have been starting to feel better not only over all my body but my belly as well (esp during orgasms). it's a long road to recovery but it can be done.

some additional things:

-make sure its not an uterine infection (or cystitis)
-make sure its not endometriosis
-make sure you do not have STD's (you can get this from just one person and even if you use a condom)
-check your diet (as odd as this sounds it's actually important). lower caffeine intake, sugars, salts, etc...
