You should be concerned. The maker of the Mirena advise that if you can feel more than the threads, or cannot feel the threads at all, you should be checked by your doctor, that it has not changed position.
When they cut the strings, if they do not cut them perpendicular but at an angle, a sharp point can develop. But you have had it for two years and I would think you would have felt that before now.
