I have oral lichen planus that I think has now progressed to skin lichen planus. What do I do now?

I have about 20 small ( the size fo your little fingernail) purple spots under/close to both of my underarms.  I think that my oral lichen planus has now gone to skin lichen planus.  I don't know any doctors that know much about it.  What type of doctor do I see and how do I find one that knows of this since only 1-2% of women over 45 get it?
