My 5 year old has a raised, crusty, quarter-sized spot on his scalp.

It is skin colored and has slowly increased in size over a year.  It doesn't itch, but it does get irritated if it gets scratched or picked at with a comb while brushing his hair.  Any idea what this could be?  No flaking, pus, redness, or "scaly" appearance.
