Can you tell if a mole is cancerous before removing it and getting it tested?

What is the first step after removing a cancerous mole? I don't have much info, but the Dr's told my Dad that this mole he had was cancerous without even getting it tested yet. Can you tell if it's cancerous just by looking at it? They did send it out for results, but I'm just wondering how do they know it cancerous alreay without getting it tested?
